Ear Trauma and Sinus Headaches?

Approximately one year ago, I incurred inner ear trauma via the insertion of cotton swab into my ear carnal. I was given oral antibiotics to treat a coexisting ear infection, which effectively cleared it up. Do you think there could possibly be fluid from the ear infection still trapped in my middle ear? (To account for the crackling sensations and/or headaches?). I'm very distressed about all of this. My doctor prescribed Allegra and Nasonex to treat supposed "sinus headaches," but I have yet to feel any degree of relief from my chronic headaches. I'm really trying to wrap my head around all of this. Should I see an ENT? I'm just not feeling all that hopeful at this point. If my ears have been structually damaged, will the headaches become longlasting? I don't know what to do. Please, please, help, if you are able to. Thank you for your time and istance. It is greatly valued and appreciated.
